CMA Physician Workforce Survey 2017 - Prize Draws

The contest is sponsored by the Canadian Medical Association (CMA), and is open to all Canadian physicians in the CMA database as of January 13, 2017, excluding employees of the CMA or its subsidiaries, and any person with whom they reside.
To enter, provide your first and last names, a valid email address, and respond to the mathematical skill-testing question on the online form following the questionnaire or here. All entrants’ names and email addresses will only be used for the draws and will not be linked to their survey responses. There are two contests. Both begin on January 13, 2017 at 12:00 a.m. ET. The first contest (Early Bird Prize) ends on February 3, 2017 at 11:59 p.m. ET and the second on February 27, 2017 at 11:59 p.m. ET. Limit: One (1) entry per person for the entire contest period. By entering you agree that you have read and understood these Contest Rules.
The prize for each contest consists of a $1000 cheque, which will be made out to the winner.
The prize must be accepted as awarded, or forfeited.
Selection of Winner
One entrant will be selected through a random draw from amongst all entries for each contest. The early bird draw will take place in Ottawa at the Canadian Medical Association on February 8, 2017 at 10:00 am ET. The second prize will take place in Ottawa at the Canadian Medical Association on March 2, 2017 at 10:00 am ET.
If the selected entrant correctly answered the skill-testing question, s/he will be informed by e-mail following the draw. To win, the selected entrant must sign a release form releasing the CMA, from any and all liability arising from participation in the contest or from the acceptance of any prize. If the selected entrant cannot be reached within 14 days of the draw, did not correctly answer the skill-testing question, or does not sign the release, another entrant will be randomly selected, and so on, until the winner has been determined. Immediately following determination of the winner, the prize will be sent to him/her. The odds of winning the prize depend on the number of eligible entries received for each contest.
Under Quebec law, the winner’s name must be made available to the Québec Régie des alcools, des courses et des jeux.
These contests are subject to applicable laws. For Quebec residents, any litigation respecting the conduct or organization of these publicity contests may be submitted to the Régie des alcools, de courses et des jeux for a ruling. Any litigation respecting the awarding of a prize may be submitted to the Régie only for the purpose of helping the parties reach a settlement.
The sponsors are not responsible for technical hardware or software failures of any kind, lost or unavailable network connections or failed, incomplete, garbled or delayed computer transmissions that may limit or interfere with a participant’s ability to take part in the contest. The sponsors reserve the right to cancel or modify the contest if fraud or technical failure threatens to destroy the integrity of the contest.
If there is a conflict between the English and French versions of these Contest Rules, the English version prevails.
