This page will guide you through the nomination process. Please note that only electronic submissions will be considered.

Step-by-step instructions

1. Decide which award you’d like to nominate someone for. Click here for descriptions of the available awards.

2. Confirm that your candidate is eligible for that award and that you are eligible to nominate them. Please note all nominees and nominators, with the exception of nominees for the Owen Adams Award of Honour, must be CMA members in good standing. Self-nomination is also not permitted, with the exception of the Dr. Brian Brodie Organizational Leadership Development Award and the Dr. Ashok Muzumdar Memorial Award for Physicians with Disabilities. For more details on the eligibility criteria for each award, please refer to their terms of reference (linked below):

3. Gather the information you’ll need to nominate your candidate. View the printable checklist for more details. In brief, here’s what you’ll need to supply:

  • full contact information for your candidate (i.e., the nominee) and for yourself
  • a nomination letter (max. two page) describing why you think your candidate deserves to win the award
  • 2 letters of support (max. two pages each)
  • your candidate’s bio or CV (max. three pages)

What happens if my candidate is not selected?

Recipients of awards will be notified in February. If the person you nominated is not selected, we’ll contact you in April and invite you to resubmit your candidate for the following year’s award.

Should fewer than three nominations be received for an award, that award will not be given out and the nomination(s) will be automatically considered for the following year.
