Organizational readiness assessment

This template guides users through an organizational readiness assessment questionnaire. It is ideally used when planning for change and is meant to give a quick snapshot of an organization’s “change readiness” at a certain point in time. Managers, supervisors, change agents and champions, if available, should each fill out and score the questionnaire.
