This booklet describes the requirements and processes for reporting adverse events and close calls, and the best approach for reviewing them. It explains how members of the Canadian Medical Protective Association and other health care provider organizations can foster a just culture of safety within their hospitals or institutions. It is suited for people in leadership/management roles as well as participants in the reporting and review process.
