Whole Day Matters Toolkit for Primary Care

The Whole Day Matters Toolkit for Primary Care offers an evidence-based guide for primary care providers who are looking to incorporate or enhance their discussions about improving physical activity, reducing sedentary time, and optimizing sleep in their adult patients. The Toolkit is available in English and French and includes a 1-page preamble, a 1-page tool, and a 1-page user guide for providers, and a 2-page handout for patients, which were designed with input from primary care providers. The Toolkit was developed in partnership with the Canadian Society for Exercise Physiology and is supported by the Public Health Agency of Canada.
